Homemaking services
Home Services include General Cleaning; Vacuuming; Dusting; Washing Floors; Washing Dishes; Laundry; Ironing; Changing Beds; Defrosting Refrigerators/Freezers; Cleaning Ovens/Stoves and China Cabinets; Caring for House Plants; Taking Out Garbage; Oversee Home Deliveries and Home Maintenance; Caring for Pets including Feeding, Exercising, and Cleaning; and much more.
Personal care
Personal Care Services include assisting the client with tasks such as Grooming (Hair Care, Shaving, Cleaning Teeth/Dentures; Makeup; Dressing) Hygiene (Tub, Bed or Sponge Baths; Showers; Toileting Activities) Mobilization (Turning, Lifts and Transfers, Exercising, Walking) Eating or Feeding, as well as Monitoring of Vital Signs such asBlood Pressures, Pulse and Respiration; and much more.
24-hour home care
24-hr home care is any combination of all services offered with Chosen Village but tailored to the individual needs hoped to be achieved by our clients and their circle of support. Services can be provided on and up to a 24-hr around the clock basis.
skilled nursing care
Skilled nursing care is provided by trained registered nurses and/ or LVN in a medical setting under a doctor's supervision. We provide the same level of nursing care clients recieve in the hospital setting. Our clients have the luxury of going from the hospital care to skilled nursing care as they continue recovering after an illness, injury or surgery in the comfort of their own home.
Transportation
Non emergency medical transportation consists of Chosen Village transporting clients to and from doctor's visits, grocery stores and light shopping, recreational activities, and other client arranged appointments.
alzheimer's care
We work with our clients to establish safe and more comfortable environments, trusting relationships and utilize techniques to maximize independence for as long as possible with clients who are struggling with Alzheimer's disease. Our specialized approach of patience and encouragement along with assistance rendered, helps our clients and their family members cope with the challenges of Alzheimer's disease as well as other dementia illnesses.
Health check services
Health Home Check is a comfort service that provides a 15-20 minute face-to-face in-home visit. It may be used for medication reminders, to note and communicate anything out of the ordinary or of concern for family peace of mind, or simply to ensure the senior is having a good day.

Specialized care offered are physical therapy, occupational therapy, speech therapy, hydration therapy, massage services and more.....
meal prep/ planning
Service involves our caregivers preparing and cooking food in the client's home or at ours. Caregivers will work with clients/families to create an appropriate meal plan, that considers all dietary restrictions. We take care of the grocery shopping and packaging serving as part of this service.
Post op Recovery Care
We provide premium medical and plastic surgery post-op/aftercare recovery services to help our clients transition in their time of need. We have experience and provide exceptional services assisting clients with tasks such as emptying drains, wound care, and monitoring their condition to ensure the best possible recovery. We are also available to do light housekeeping, prescription pickup, medication reminders, grocery shopping, and other errands, as needed. We come our clients, so they can recovery at home. Our service plans are tailored to the individual needs of each client.
outdoor/ Lawn care
Service involves lawn care such as cutting lawns, pulling weeds, raking leaves, trimming shrubs, and tending to gardens. On occasion we may need to shovel snow, Split and stack firewood, etc. We do what is necessary to maintain care for the lawns of the clients we serve.
